If this is your Initial Appointment:

  • I look forward to meeting you! Please keep an eye out for the initial paperwork that you will receive via email- there are three forms:
       1) HIPAA Notice of Privacy Practice
       2) Assumption of Risk and Release of Liability
       3) Initial Assessment Form
  • If you complete them before our session, we will have more time for education and counseling.
  • If you would prefer to fill out the Initial Assessment form with me as part of our session, we can certainly do that. However, I ask that you submit a $35 deposit to hold your session. You can do so through the digital invoice you’ll receive for the session ahead of time. If you do fill out all of your paperwork, there is no deposit required.

If this is a Follow-Up Appointment:  If you think of any topics you’d like to explore more before our next session, definitely reach out and let me know. I am always available via the Practice Better app, phone, or email to answer any questions in between sessions, as well.
